Office Policies

1. A yearly physical is needed to remain an active patient.
2. This is a professional office; we reserve the right to discharge patients for disrespectful behavior.
3. Prescriptions have a 48 hour turn around period.
4. Prior authorizations can take 2-3 weeks for insurance determinations.
5. Blood work, procedures and services are not necessarily covered by your insurance. Please contact your insurance for details. We cannot guarantee coverage.
6. Same day appointments are offered on a first come first serve basis. If no appointments are available you will be directed to an urgent care.
7. Failure to pay account balances may result in patient discharge.
